HIGH-5 DIVISION: 9-22 yr. old (must be 9 by 9/1/08; cannot be 23 before 9/1/08)
Needed at registration:
(1) New HIGH-5 REGISTRANTS: Birth Certificate
(2) New HIGH-5 REGISTRANTS: need a medical clearance letter from their physician to
participate. (Limitations and restrictions of activities should be noted.)
(3) Child must be present at the time of registration.
(4) Parent or guardian present to sign registration forms.
(5) $50.00 REGISTRATION FEE (Payable to: High Five Basketball)
