Players must be 9 - 22 years old (must be 9 by 9/1/09 and cannot be older than 22 on 9/1/09)

RETURNING PLAYERS ONLY: REGISTRATION REQUIREMENTS:

Child MUST be present at the time of registration (NO EXCEPTIONS).
Parent or guardian MUST be present (NO EXCEPTIONS).
2009 Registration Form filled out at Registration.
$50.00 REGISTRATION FEE (Payable to: High-5 Basketball, Inc.)
Parent Responsibility Form.
PLEASE NOTE: We are NOT requiring Medical Clearence Letters for Returning Players!

NEW PLAYERS: REGISTRATION REQUIREMENTS

Copy of Birth Certificate (NEW PLAYERS ONLY).
New HIGH-5 REGISTRANTS need a medical clearence letter DATED 2009 from their physician to participate. **Limitations and restrictions of activities should be noted.
** (No 2009 medical form, no registration. NO EXCEPTIONS.) **
2009 Registration Form filled out at Registration.
