THINGS TO KNOW
The High-5 Division is a division of the Boca Hoops Recreation Basketball Program. This division is designed to enable the child with disabilities to participate in the game of basketball. Each child of this division has a B-Ball Buddy that works with that child to enable them to learn the skills of basketball and to learn sportsmanship and teamwork. The athletes with disabilities will experience the enjoyment of the game of basketball as part of the Boca Hoops Family. The High-5 Division will run during the regular Boca Hoops Season, August through November.

1) Participants must be eight (9) years old by September 1st, and no participant can be sixteen (22) years old by September 1st.
2) Participants need a letter of medical clearance from "their" physicians to participate. (Limitations and restrictions of activities should be noted.)
3) Must be committed to playing the entire Boca Hoops season.
2008 REGISTRATION INFORMATION:
HIGH-5 Basketball – 9 - 22 years old (must be 9 by 9/1/08; cannot be older than 22 on 9/1/08). (Please note that we have raised the starting age from 8 to 9 years old).

RETURNING PLAYERS ONLY: REGISTRATION REQUIREMENTS
(1) Child must be present at the time of registration (NO EXCEPTIONS).
(2) Parent or guardian must be present (NO EXCEPTIONS).
(3) 2008 Registration Form filled out at Registration.
(4) $50.00 REGISTRATION FEE (Payable to: HIGH-5 Basketball, Inc.)
(5) Please note that we are not requiring MEDICAL CLEARANCELETTERS FOR RETURNING PLAYERS!!)
NEW PLAYERS: REGISTRATION REQUIREMENTS
(1) Copy of Birth Certificate (New Players Only)
(2) New HIGH-5 REGISTRANTS need a medical clearance letter DATED 2008 from their physician to participate.
**Limitations and restrictions of activities should be noted.
**(No 2008 medical form, no registration. NO EXCEPTIONS.)**
(3) 2008 Registration Form filled out at Registration.
(4) Child must be present at the time of registration (NO EXCEPTIONS).
